Favoring the direction of nursing care to be provided to these patients, especially in emergency care units, helps to reduce complications and promote adequate care for the elderly. Objective: to develop and evaluate a nursing care protocol for elderly patients with femur fractures in an emergency care unit. Methodology: This is a methodological study in two stages, namely: 1- Diagnosis, indicators, objectives, approval: in this stage, the medical records of elderly patients treated with femur fractures in the unit were searched and a questionnaire was applied among nursing professionals to identify the care provided to these patients; 2- Development and evaluation of the protocol: the protocol was developed and evaluated by specialists and nursing professionals working in the unit. The study was approved by the Research Ethics Committee (CAAE: 64244522.4.0000.5243; opinion number: 5.860.801). All ethical aspects were considered. Results: Twenty-two (100%) medical records of elderly individuals treated for femur fractures were consulted, of which 14 (63.6%) were women, 16 (72.7%) were over 70 years of age, and 21 (95.5%) were from a city in the central-west region of Minas Gerais. Femoral neck fractures were the most common (12 (54.5%), with falls being the main cause (18 (82.0%)), and the place where the injury occurred (15 (68.2%). Regarding the nursing care provided to these patients, the following stood out: repositioning in bed 39 (93%), bed bath 41 (97.6%), peripheral venous access puncture 42 (100%), diaper change 42 (100%) and care with indwelling bladder catheterization 39 (93%). In the analysis of nursing care provided among nursing professionals, the sample consisted of 42 participants, 34 (81.0%) women, with an average age of 37 years, predominance in the age range of 31 and 39 years (14 (33.3%), minimum age of 23 years and maximum of 59 years. Regarding the professional category exercised in the unit, 15 (35.7%) nurses and 27 (64.3%) nursing technicians. It was identified through the word cloud that “patient” assumes the centrality, referring to the care provided. The team pays attention to the patient's risk of falling, as well as their bed rest and the need to help them with their daily needs and with changing their position. The similarity method also verified the centrality of the patient, which presents some main branches with more prominent words such as change and decubitus, which have in their thinner branches the consequence of not performing this procedure. In the Descending Hierarchical Classification, the five classes generated address issues related to guidance to the patient and companion, prevention of iatrogenesis, teamwork focused on the patient and patient safety. Based on the information obtained in the previous stage, the protocol was developed containing guidelines on the care provided to patients with femur fractures divided into six topics, namely: repositioning in bed, bed bath, measurement of vital signs, puncture and care with venous access, diaper change and care with indwelling urinary catheter and emptying of collection bag. Each item contains the important aspects of each procedure, materials necessary for its performance and the description of the execution of the procedure. In the evaluation of the protocol, a CI of 1.0 was obtained among the experts and a CI of 1.0 among the target audience. Regarding the results of the evaluators, the protocol presented satisfactory functionality, usability, efficiency, audiovisual technique, environment and procedure in more than 80%. Conclusion: The protocol was constructed and validated with excellent results. It is expected that this instrument will be implemented in the unit in order to standardize the care provided to these individuals, ensuring quality of nursing care provided. In addition, this study can contribute to raising awareness among the nursing team about the importance of their role in the rehabilitation and health recovery of elderly patients with femur fractures.
