A non-pharmacological protocol can support the implementation of systematic recommendations to treat urinary incontinence in this population. This study aimed to develop a physical and behavioral protocol to treat urinary incontinence in older women and have it validated by an expert panel. This methodological study adopted a qualitative approach to structure and develop a protocol based on the literature. A focal group was used for the experts to validate its content, review and modify its items, and determine its definitions. The protocol was based on a theoretical framework and a comprehensive scientific and technical literature review. The following criteria were adopted to develop and assess the protocol: origin, objective, development group, conflict of interest, evidence, review, flowchart, result indicator, validation by technical professionals, limitations, and implementation plan. It comprises the topics: 1) Education on urinary incontinence; 2) Behavior assessment using a voiding diary; 3) Identification of urinary loss using the pad-test; 4) Proprioception; 5) Pelvic floor muscle strength assessment; 6) Perineal exercises. The protocol’s operational procedure indicates that it must be implemented during 12 weeks according to four stages. In the first stage, older women must be characterized (urinary incontinence /No urinary incontinence) using the International Consultation on Incontinence Questionnaire – Short Form. In the second stage, the women identified with urinary incontinence, whose functional performance (verified through the timed up-and-go test) is preserved, and who have an accessible telephone, participate in a lecture on urinary incontinence. The third stage consists of consultations and exercises to strengthen the pelvic floor muscle and is distributed in 12 weeks: Month 1, Month 2, and Month 3. The fourth stage includes the reassessment and comparison of baseline pelvic floor muscle strength and health-related quality of life. Five specialists, 45.2 years on average, four women and four married, validated the protocol. Three were physical therapists, one was a nurse, and one was a physician. Most had graduated more than ten years ago, with a background and professional experience in the urinary incontinence field. Even though most items obtained the agreement and/or were considered relevant by most experts in all criteria, we sought to meet most of the changes proposed. The protocol was reformulated, and the changes proposed by the experts were incorporated. As a result, the protocol’s content and presentation were considered valid and suitable for implementing physical and behavioral interventions to treat older women with urinary incontinence.
