dc.description.resumoObjetivo: O objetivo desse estudo clínico prospectivo foi avaliar, por meio de imagens por ressonância magnética, as possíveis alterações na posição e forma do disco das articulações temporomandibulares em crianças e pré adolescentes tratados com dois protocolos distintos de expansão rápida da maxila e máscara facial. Métodos: A amostra de conveniência foi composta por 88 pacientes, de ambos os gêneros, má oclusão Classe III ou Classe III Subdivisão, com idades entre 6 e 13 anos, selecionados no Departamento de Ortodontia da Universidade Santa Cecília e divididos em 3 grupos. O grupo 1 foi composto por 34 pacientes, sendo 8 meninos e 26 meninas tratados com expansão rápida da maxila, seguido de terapia com máscara facial (11,41±3,13 meses). O grupo 2 foi composto por 34 pacientes, sendo 18 meninos e 16 meninas, tratados com expansão rápida da maxila conforme o protocolo alternativo modificado de expansão e constrição rápida da maxila seguido de terapia com máscara facial (13,26±2,77 meses). Esses grupos tratados foram distribuídos aleatoriamente com uma taxa de alocação de 1:1 de acordo com os dois protocolos de tratamentos. Os outros 20 pacientes do grupo 3 Grupo Controle não tratados, 9 meninos e 11 meninas, com média de idade 7,83±1,39 anos foram acompanhados(11,35±2,98 meses). As imagens das articulações temporomandibulares por ressonância magnética foram obtidas no momento (M1) antes e (M2) após o final do tratamento ou período de acompanhamento. Foram realizados teste de McNemar, teste exato de Fisher e concordância intra e interobservador (K) (p ≤ 0,05). Resultados: Não houve diferença estatisticamente significante nas variáveis cefalométricas basais em M1 entre os grupos. Houve diferença estatisticamente significante entre os grupos (p< 0,001) em relação à forma do disco em M1, sendo que G1 apresentou maior ocorrência de forma alterada em comparação ao G2. Em todos os grupos estudados foram observadas diferenças não significantes entre M1xM2 quanto à posição e forma do disco. Conclusão: Pode se concluir que durante o período de estudo os protocolos de tratamento empregados não resultaram em alterações significativas na posição e forma do disco das articulações temporomandibulares avaliados por meio de RM em crianças e préadolescentes com má oclusão de Classe III ou Classe III Subdivisão. dc.description.abstractObjective:The aim of this prospective clinical study was to evaluate, using magnetic resonance imaging, possible changes in the position and shape of the temporomandibular joint disc in children and pre-adolescents treated with two different rapid maxillary expansion and facemask protocols. Methods: The convenience sample consisted of 88 patients, of both genders, Class III or Class III Subdivision malocclusion, aged between 6 and 13 years, selected at the Department of Orthodontics of University Santa Cecília and divided into 3 groups.Group 1 consisted of 34 patients, 8 boys and 26 girls treated with rapid maxillary expansion, followed by face mask therapy (11.41±3.13 months). Group 2 consisted of 34 patients, 18 boys and 16 girls, treated with rapid maxillary expansion according to the modified alternative protocol of rapid maxillary expansion and constriction followed by face mask therapy (13.26±2.77 months). These treated groups were randomly distributed with a 1:1 allocation ratio according to the two treatment protocols. The other 20 untreated patients from group 3 - Control Group, 9 boys and 11 girls, with a mean age of 7.83 ± 1.39 years were followed up (11.35 ± 2.98 months). Temporomandibular joints were obtained by magnetic resonance imaging before (M1) and after (M2) a period of treatment or follow-up.
